About The Position

The Housing Case Manager II (HCM) has demonstrated proficiency meeting and maintaining productivity and documentation targets and will provide Medicaid Billable outreach, engagement, intake, housing search/placement and on-going support services to homeless persons with a Georgia Housing Voucher placed in scattered site housing in the community. The HCM II will be a member of a Housing First team formed to expand Step Up’s successful field-based permanent supported housing model in the Atlanta area. The team will provide in-person housing support services to adults experiencing chronic homelessness and co-occurring mental health that leads to permanent supported housing. This position fully promotes the use of Housing First, Harm Reduction, Trauma-Informed Care and the Motivational Interviewing evidenced-based models of Recovery Oriented service delivery.

Requirements

Bachelor’s degree from an accredited college or university or Associate’s degree and the minimum 2 years of human services experience as listed in the required experience section below.
A minimum of 2 years related human services experience providing case management services serving the following populations: chronically homeless, adults struggling with a mental illness.
Minimum of 2 years providing social services case management which included collaborative client-centered support that assessed needs, established care plans, and connected individuals to vital community resources guiding clients through complex systems to promote long-term stability, safety, and independence.
Experience writing structured progress notes (GIRP or similar note format) and documenting services delivered timely.
Georgia Driver’s License with a good driving record and the ability to pass background checks.

Nice To Haves

Experience delivering Medicaid billable and/or social service case management services in client’s home and/or other field-based community settings.
Knowledge of State and/or federally funded housing voucher programs
Familiarity with meeting specific productivity standards preferred.
Proficient use of Office 365 products required.

Responsibilities

Provide Medicaid Billable intensive case management Housing First services to a caseload of 25-40 chronically homeless persons to obtain and maintain Permanent Supportive Housing. Services include: outreach and engage referrals, conduct intake and assessment of barriers, document acquisition, service plan development, linkages and referrals to clinical and appropriate community resources, housing search, leasing up, moving into housing and on-going in-home services.
Meet with assigned members as outlined in the Treatment Plan utilizing telehealth and in-person home visits to review progress toward goals and provide Medicaid billable case management and skill building interventions that increase benefits, vocational and independent living skills; and collaborating with Step Up team, community partners and property management staff to ensure housing stability.
Able to provide ongoing support and crisis intervention independently and under the supervision of the Team Leads or Program Manager as needed to address symptoms as defined in the assessment and on treatment plan, to improve level of functioning, to support mental health management and to promote self-sufficiency.
Comprehensive coordination of care including developing/maintaining relationships with community treatment providers to ensure ongoing clinical/substance use treatment supports and successful transition to and maintenance of independent living.
Provide transportation using Lyft, team vehicles or public transportation and accompany the client when necessary to teach independent living skills.
Using the GIRP note format, document all services provided to members and collateral contacts on behalf of members as required by Medicaid billing standards, DBHDD and Step Up in Welligent electronic health record within proscribed timeframes outlined in SUOS Documentation and Productivity standards Policy.
Responsible for monitoring and documenting interventions related to housing needs, bridge funding, lease up packages, lease renewals, client forms and charts, tracking productivity and documentation for all services, and client Welligent file maintenance.
Meet weekly, monthly, and quarterly Medicaid productivity targets and billing quotas as presented by Program Manager and outlined in Documentation and Productivity standards policy.
Process all paperwork in a timely and accurate manner.
Work collaboratively with members of the Step Up team and community providers to effectively perform their assigned job duties and contribute to the achievement of the program's performance and fidelity goals and objectives of each client’s housing stability.
Attend staff meetings, supervision and training as required. Other duties as assigned.
