About The Position

LSI's Services for People with Disabilities is seeking a dedicated and energetic full-time supervisor to work as a Contract Monitor for their Host Home Services in the Eastern Iowa regions. This position will be centralized with homes in the Cedar Rapids, Cedar Valley, or Dubuque areas. The role offers some work-from-home flexibility and involves travel within specified counties (Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, Winneshiek) and potentially outside these counties to cover service needs and meetings. The Iowans served by this program are seeking greater confidence and independence, and LSI aims to empower them toward success.

Requirements

  • HS Diploma/GED and five years of related experience OR Associates Degree and three years of related experience OR Bachelor's Degree and one year of related experience.
  • Valid driver's license.
  • Auto insurance.
  • Good driving record.
  • Ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.

Nice To Haves

  • Bachelor's Degree in a human services related field and one year of related experience.
  • Years of related experience in a supervisory or leadership role.

Responsibilities

  • Oversees specified Host Homes in the Eastern Iowa service area.
  • Responsible for the oversight of services and communicating with interdisciplinary team members.
  • Oversees contractual compliance and ensures assigned independent providers deliver quality care and meet contractual requirements.
  • Acts as the main contact for interdisciplinary team members and for individuals served in the host home program.
  • Works directly with assigned host homes to provide support, assistance, and instruction to facilitate personal growth and independence.
  • Facilitates and participates in interviews and evaluations related to the contracting process for independent providers.
  • Participates in an on-call rotation.
