Host Home Provider - Southeast Iowa Region
About The Position
LSI is seeking passionate and independent individuals to become Host Home Providers in the Southeast Iowa region. This role involves opening your home to an adult with disabilities (including intellectual, physical, mental health, and brain injuries) and providing person-centered care. You will guide, support, and mentor the individual, helping them achieve goals such as shopping, budgeting, cooking, personal hygiene, and community integration. This is a contracted, independent provider position where you'll build a meaningful relationship and integrate the individual into your family life. LSI provides matching services, support, guidance, and training resources. A home safety check and training are required before placement.
Requirements
- Must be 21 or older.
- Must reside in Iowa.
- Must have a high school diploma or GED.
- Must have a current/valid driver's license.
- Must meet minimum insurance requirements.
- Must have access to a computer/internet for documentation.
- Must successfully complete all steps of the screening, interview, and matching process.
- Must have flexibility and creativity.
- Must have a passion for making a difference.
- Must have auto insurance.
- Must have commercial general liability insurance.
- Must have a decent driving record.
- Must pass an extensive background check (including Dependent Adult and Child Abuse registries, Sex Offender registry, criminal history check, reference checks, and home safety check).
- All individuals over 18 residing in the home must pass background checks.

Responsibilities
- Provide guidance, support, and care for an individual with disabilities living in your home.
- Work with the mentee on goals such as shopping, budgeting, coping skills, cooking meals, personal hygiene, and community integration.
- Act as an advocate and mentor to help the individual live their best life.
- Integrate the individual into your family and community.
- Complete documentation requirements using a computer and internet access.
- Pass an extensive background check, including checks of Dependent Adult and Child Abuse registries, Sex Offender registry, criminal history, and reference checks.
- Ensure all individuals over 18 residing in the home pass background checks.
- Pass a home safety check.
