About The Position
The Licensed Practical Nurse performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider. This position is in the Med/Surg/Detox department at Gouverneur Hospital and is per diem, with variable 12-hour shifts.
Requirements
- Graduate of an accredited practical nursing program.
- Current, unrestricted New York State Licensed Practical Nurse license.
Nice To Haves
- One year of post-licensure practice as a Licensed Practical Nurse.
Responsibilities
- Collect patient data related to patient and family needs.
- Perform tasks related to the comprehensive plan of care involving the patient and family.
- Perform and document direct patient- and family-centered care according to nursing standards of practice.
- Respond to patient conditions to achieve positive patient outcomes.
- Perform tasks related to discharge planning to ensure a safe and appropriate transition of care.
- Supervise unlicensed team members in the delivery of care.
- Provide world-class customer service to patients and families.
