About The Position
Benchmark is seeking a compassionate Per Diem Licensed Practical Nurse (LPN) to provide nursing care to residents in a warm, comfortable, home-like environment. The LPN works with a supportive team and helps residents maintain independence while coordinating and overseeing their care.
Requirements
- Current state LPN license and CPR certification.
- Graduate of an approved LPN program, as required by the state.
Nice To Haves
- One to two years of experience working as a nurse is preferred.
- Previous experience working with elderly people in a paid or volunteer position is highly desirable.
Responsibilities
- Accurately assess, communicate, and document residents’ status.
- Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director as required by state regulations.
- Direct and supervise the daily work assignments of Resident Care Assistants.
- Mentor, guide, and direct Lead Resident Care Assistants in their job responsibilities.
- Coordinate care needs with community providers through an effective case management process.
- Balance service demands with support for resident independence.
- Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources as required by state regulations.
- Respond to emergencies and personal emergency response system alerts, and determine when backup is needed.
