About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment at Plymouth Crossings. The role is part of a supportive team focused on building community and long-lasting relationships with associates and residents. Flexible per diem hours are offered, with pay of $34–$38 per hour.

Requirements

• Current state LPN license.
• CPR certification.
• Graduate of an approved LPN program, per state requirements.

Nice To Haves

• A minimum of 1 to 2 years of experience working as a nurse is preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director, in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with supporting resident independence.
• Recognize and assist in preventing elder abuse, neglect, and exploitation, and report to appropriate sources per state regulations.
• Respond to emergencies and personal emergency response systems, and know when to call for backup.
