Responsibilities

• Build and grow referral relationships with area hospitals, case managers, discharge planners, and physician groups.
• Drive census by converting referrals into admissions.
• Represent the facility at hospital rounds, meetings, and community events.
• Track referral sources and follow up with them.
• Work with admissions and clinical teams to ensure referrals convert smoothly.
