Hospital at Home RN Case Manager
About The Position
St. Luke’s is hiring a Hospital at Home RN Case Manager to provide patient-centered care through an innovative alternative to traditional inpatient care. The Hospital at Home model offers acute-level care to patients in their own homes rather than in the hospital, with the aim of enhancing the patient experience, reducing costs, and improving outcomes. The position is located at the Virtual Care Center at Boise St. Luke’s Plaza. The role includes intensive onboarding and training. Requirements
- Associate’s degree or diploma in nursing, or a Bachelor’s degree in nursing.
- Two years of RN experience.
- Current RN license in Idaho.
