Hospice Visit RN - Weekends (Paid 1.5x)
About The Position
The Weekend Option Registered Nurse provides nursing services to assigned patients, including direct patient care and collaboration with interdisciplinary team (IDT) members and other healthcare providers in various care settings. Care must promote an interdisciplinary approach, respect patient and family goals, meet Medicare and Medicaid participation conditions and accreditation requirements, and reflect TRU Community Care’s mission and philosophy. This position works every weekend, Saturday and Sunday from 8 a.m. to 8 p.m., completing at least two 12-hour shifts (24 hours per week) while receiving pay for 36 hours. The role is field-based, with travel to multiple locations according to patient needs. The nurse primarily conducts after-hours home visits and provides care as delegated by the Phone Triage Nurse, with cross-training to assist the phone triage team as needed. The listed hourly pay range is $42.34 to $50.81. Mileage is reimbursed at $0.76 per mile.
Requirements
- Current RN license in the state of Colorado.
- At least one year of RN experience.
- Successfully pass required background checks and drug screening.
- Travel to multiple work locations.

- Hospice or home health experience is highly preferred.
Responsibilities
- Conduct after-hours home visits and provide patient care as delegated by the Phone Triage Nurse.
- Provide nursing care in accordance with each patient’s hospice plan of care.
- Cross-train on phone triage and assist the Triage RN team as necessary.
- Identify, communicate, and collaborate on patient problems, priorities, and issues based on assessment data.
- Establish measurable patient goals and communicate an ongoing plan of care to the interdisciplinary team.
- Recognize changes in patients’ clinical status and report findings to the appropriate team member.
- Collaborate with patients and families when developing the plan of care, adjusting communication style as needed.
- Work every weekend, completing at least two 12-hour shifts (24 hours) while being paid for 36 hours.
