Hospice Visit RN - Weekends (Paid 1.5x)
About The Position
TRU Community Care is seeking a Weekend Option Registered Nurse to provide nursing services to assigned hospice patients. The RN delivers direct patient care, works with the interdisciplinary team (IDT) and other health care providers in various care settings, and supports care that respects patient and family goals, meets Medicare/Medicaid participation and accreditation requirements, and reflects TRU Community Care’s mission and philosophy. The role primarily involves after-hours home visits, with care delegated by the Phone Triage Nurse. The nurse must work every weekend, completing at least two 12-hour shifts (24 hours) during the weekend schedule while being paid for 36 hours. The stated schedule is 8 a.m. to 8 p.m. on Saturday and Sunday. The position is field-based, with locations varying across the territory according to patient needs; mileage is reimbursed at $0.76 per mile.
Requirements
- Current RN license in the state of Colorado.
- At least one year of RN experience.
- Successfully pass required background checks and drug screening.
- Ability to travel to multiple work locations.
Nice To Haves
- Hospice or home health experience is highly preferred.
Responsibilities
- Conduct after-hours home visits and provide care to patients as delegated by the Phone Triage Nurse.
- Provide nursing care that meets patient needs and follows the patient’s hospice plan of care.
- Cross-train on phone triage and assist the Triage RN team as necessary.
- Identify, communicate, and collaborate on patient problems, priorities, and issues based on assessment data.
- Establish measurable patient goals and communicate an ongoing plan of care to the interdisciplinary team.
- Recognize changes in patients’ clinical status and report findings to the appropriate team member.
- Collaborate with patients and families when developing the plan of care, adapting communication style as needed.
- Work every weekend and complete at least two 12-hour shifts (24 hours) within the weekend schedule.
