About The Position

Emblem Hospice is hiring a full-time Hospice RN to serve as a trusted case manager for patients in their homes and communities throughout the South Scottsdale area. The nurse delivers compassionate, skilled care, helps patients and families make care decisions, and supports comfort and dignity. The position requires travel and follows a Monday-Friday schedule.

Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona.
• Minimum of 2 years of nursing experience.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.

Nice To Haves

• Hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, but not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising them as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
