About The Position
Constellation Health Services is seeking a compassionate and skilled Hospice Registered Nurse Case Manager to care for patients and their families in homes, assisted living facilities, and skilled nursing facilities. The role covers Norwalk, Stamford, and Greenwich, Connecticut. The Case Manager provides skilled nursing care, coordinates patient care with the interdisciplinary team and caregivers, monitors and reports changes in patient status, and completes required clinical documentation. Weekend and holiday coverage may be needed.
Requirements
- Graduate of an accredited RN program.
- Hold a Registered Nurse license in the state of employment.
- At least 1–2 years of RN experience.
- Basic knowledge of the iOS operating system.
Nice To Haves
- Previous hospice care experience is strongly desired.
Responsibilities
- Provide skilled nursing care, including administering medications and treatments prescribed in the physician's plan of care.
- Perform comprehensive patient assessments and coordinate the plan of care with relevant disciplines, family members, and primary caregivers.
- Observe and evaluate patients' physical and emotional status, and report changes to the nursing supervisor and attending physician.
- Participate in weekly Interdisciplinary Team Meetings and provide updates on patients' physical, emotional, psychosocial, and spiritual status.
- Maintain and process documentation in accordance with agency standards.
- Use a computer-based documentation system and submit required clinical documentation, time records, and other information within required time frames.
- Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluation.
- Provide weekend and holiday coverage as needed.
