About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The position includes on-call responsibilities as assigned.
Requirements
- Current RN license specific to the state or states assigned.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- At least 1 year of nursing experience.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately and on time, in accordance with care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
