About The Position
Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA. This position offers the opportunity to make a meaningful impact by providing compassionate end-of-life care and support to patients and their families in the comfort of their homes.
Requirements
- Active Pennsylvania RN license
- Strong clinical and organizational skills
- Excellent communication and compassionate patient care abilities
- Valid driver’s license and reliable transportation
Nice To Haves
- Hospice or home health experience preferred
Responsibilities
- Manage and coordinate hospice patient care plans
- Perform comprehensive nursing assessments
- Provide symptom management and patient education
- Collaborate with physicians, caregivers, and interdisciplinary team members
- Maintain accurate and timely documentation
- Support patients and families emotionally throughout the hospice journey
