About The Position
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Allentown area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.
Requirements
- Current Pennsylvania RN license (required)
- Minimum 1 year of nursing experience (hospice, home health, or case management preferred)
- Strong clinical, organizational, and communication skills
- Compassionate, patient-focused approach to care
- Valid driver’s license with reliable transportation

Responsibilities
- Manage and coordinate care for an assigned hospice caseload
- Perform comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing visits in patients’ homes and other care settings
- Collaborate with physicians, social workers, chaplains, aides, and other team members
- Educate patients and families on disease progression, symptom management, and medications
- Monitor patient status and adjust care plans as needed
- Ensure accurate, timely clinical documentation in accordance with hospice regulations
- Participate in interdisciplinary team meetings and care planning
- Serve as a patient and family advocate throughout the hospice journey
