Requirements
- Active Pennsylvania RN license
- Excellent communication and organizational skills
- Compassionate, patient-centered approach
- Valid driver’s license and reliable transportation
Nice To Haves
- Hospice or home health experience preferred
Responsibilities
- Coordinate and manage hospice care for your assigned patients
- Perform comprehensive patient assessments and develop individualized care plans
- Collaborate with physicians and the interdisciplinary hospice team
- Provide compassionate education, guidance, and support to patients and families
- Maintain accurate and timely documentation in accordance with hospice regulations
- Build meaningful relationships with patients and families during an important time in their lives
