About The Position

Reliance Hospice and Palliative Care is hiring a full-time Hospice Registered Nurse to provide hospice and palliative visits in the Torrance area and surrounding communities. The nurse plans, organizes, and directs hospice care, providing direct professional nursing services to meet the needs of patients and families, with an emphasis on community health education and experience. The hourly rate starts at $52 and is negotiable based on years of experience. The RN assesses patients and families, develops and implements individualized plans of care, provides prescribed treatments and medications, coordinates care, and documents clinical status and services. The position includes working with an interdisciplinary group, participating in on-call duties, supervising hospice aides, and traveling to patients. The role requires physical activities including walking or standing for prolonged periods, lifting and transferring patients and equipment, and bending, reaching, kneeling, or crouching.

Requirements

Current California RN license to practice professional nursing.
Graduate of a National League for Nursing-accredited school of nursing.
Current CPR certification.
Driver's license and reliable automobile with California insurance.
At least two years of hospice and palliative medicine experience is required.
Self-directed and able to work with minimal supervision.
Excellent observation, problem-solving, verbal and written communication, and nursing skills as measured by a competency checklist.
Ability to organize and prioritize workload independently.
Ability to walk or stand for prolonged or considerable periods; lift, position, or transfer patients; lift supplies and equipment; and perform considerable reaching, stooping, bending, kneeling, or crouching.
Visual acuity and hearing sufficient to perform required nursing skills.
Ability to commute to Torrance, California.

Nice To Haves

At least one year of home care or hospice nursing experience is preferred.
Spanish language ability is preferred.

Responsibilities

Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
Provide physical assessments and histories of current and previous illnesses.
Provide professional nursing care using all elements of the nursing process.
Assess and evaluate patient status, and write and initiate plans of care.
Develop care plans with nursing goals and palliative nursing actions, including patients and families in the planning process.
Regularly reevaluate patient and family or caregiver needs.
Initiate appropriate preventive and rehabilitative nursing procedures.
Administer medications and treatments prescribed by physicians in the plan of care.
Assist patients with activities of daily living and support self-sufficiency and comfort care.
Act as case manager when assigned by a clinical or nursing supervisor and coordinate care for an assigned caseload.
Complete, maintain, and submit accurate clinical notes about patient condition and care provided.
Record pain and symptom management changes and outcomes as appropriate.
Communicate patient needs and changes in condition to physicians, and obtain or receive physician orders as required.
Teach patients and families or caregivers self-care techniques as appropriate.
Provide physician-ordered medication, diet, and other instructions, and use opportunities for health counseling.
Work with the interdisciplinary group and attend its meetings.
Participate in on-call duties as defined by the on-call policy.
Ensure arrangements for equipment and other necessary items and services are available.
Maintain professional growth and skills through continuing education and in-service classes.
Supervise hospice aides.
Perform other duties as assigned.
