About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice Registered Nurse provides compassionate, patient-centered care by assessing patient needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team. The position is salaried with mileage and follows a daytime schedule of Saturday, Sunday, and Monday, with three 12-hour shifts. Benefits begin on the first of the month after hire.

Requirements

Current RN license specific to the state or states where assigned to work.
Valid driver’s license, reliable transportation, and liability insurance.
Current CPR certification.

Nice To Haves

At least 1 year of nursing experience.

Responsibilities

Perform initial nursing assessments on new admissions.
Complete, coordinate, and revise the total plan of care, maintaining continuity of patient care through collaboration with appropriate staff.
Regularly communicate patient progress to the clinical manager, physician, and care team.
Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
Record changes and outcomes in pain and symptom management as appropriate.
Educate families about the patient’s condition, needs, and care.
Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Perform other duties as assigned.
