About The Position

Puget Sound Home Health & Hospice is seeking an experienced, full-time after-hours Hospice Registered Nurse (RN) Case Manager to provide high-quality hospice nursing care across Thurston, Grays Harbor, and Mason counties in Washington. The role works an alternating 7-on/7-off schedule, with coverage from Monday at 5:00 PM to Friday at 8:00 AM and Friday at 5:00 PM to Monday at 8:00 AM.

This autonomous, high-impact clinical role serves patients and families facing advanced illness. The RN provides after-hours visits and urgent care interventions, completes skilled assessments and in-person visits as assigned by the Triage RN, manages pain and symptoms, educates patients and families, documents care, and coordinates with the interdisciplinary hospice team. Compensation is $90,000–$110,000 annually, depending on experience.

Requirements

• Current Washington State Registered Nurse (RN) license.
• Hospice agency experience.
• Current AHA BLS certification.
• Strong clinical assessment and independent decision-making skills.
• Ability to work autonomously during evening, night, and early-morning hours.
• Reliable, insured vehicle and valid driver’s license.

Responsibilities

• Deliver after-hours hospice nursing visits, urgent care interventions, and skilled assessments.
• Complete in-person visits as required and assigned by the Triage RN.
• Provide pain and symptom management in accordance with hospice standards of care.
• Educate patients and families on hospice philosophy, medications, and care expectations.
• Document assessments and interventions accurately and efficiently.
• Communicate promptly with the interdisciplinary hospice team to ensure continuity of care.
• Participate in QAPI and quality improvement initiatives.
