Hospice Registered Nurse RN - Case Manager
About The Position
Otterbein, a not-for-profit, faith-based ministry, is seeking a Hospice RN Case Manager to support the growth of its Hospice ministry. Hospice care focuses on helping people facing life-limiting illnesses experience peace, comfort, and quality of life while supporting their families and caregivers. Otterbein Hospice provides care in patients’ homes and involves patients and loved ones in developing personalized plans of care, with support from a team that includes nurses, a chaplain, and a social worker. The Hospice RN Case Manager is responsible for a primary caseload and provides skilled professional nursing care to patients, families, and caregivers as prescribed by the physician. Care must comply with the Hospice Medicare Conditions of Participation, Ohio hospice licensure laws, and agency policies and procedures. The role is full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m., with an on-call and weekend rotation.
Requirements
- Graduate of an approved nursing program.
- Valid Ohio Registered Nurse (RN) license.
- A minimum of 2–4 years of direct patient care experience in hospice and case management experience is preferred.
- Experience with end-of-life diagnoses is preferred.

Responsibilities
