About The Position

Sage Group HealthCare Staffing is seeking a Hospice Registered Nurse Case Manager for a travel/contract position in Milford, Connecticut. This full-time role involves providing and coordinating care for assigned hospice patients in their homes. The position focuses on developing and implementing nursing care plans, supporting patient recovery, and empowering patients and families through education on disease management. The nurse will provide hospice nursing care within established standards and protocols, assess patients ongoing, document visits within 24 hours, and notify physicians of significant changes in patient status. The role also includes providing bereavement support to families.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Hospice Nursing experience in a home health setting: 1 year within last 3 years - Required
  • Home Care Home Base (HCHB) charting experience - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide medical or religious exemption
  • ASN or an equivalent – Required
  • RN License for State of OH or Compact - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned hospice patients in comfort of their homes.
  • Develop and implement nursing care plans including Start of Care and admissions as assigned.
  • Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
  • Provide bereavement support to patient’s families as needed.
