About The Position

Sage Group HealthCare Staffing is seeking an RN Hospice for a 13-week travel/contract position in Montrose, CO. This role involves providing nursing care for hospice patients in various settings including private homes, nursing homes, group homes, and assisted living facilities. The nurse will manage an assigned caseload, lead the interdisciplinary team, and be responsible for assessments, care planning, pain and symptom management, and administering physician-prescribed medications. Accurate and timely patient documentation and charting are essential. The position covers Montrose, CO, and surrounding areas.

Requirements

• Registered Nurse: 1 to 2 years - Required
• Hospice Nursing experience: 1 to 2 years – Required
• EMR/EHR charting - Required
• Must have valid Driver’s License and Personal Auto Insurance
• ASN or an equivalent – Required
• RN License for State applying or Compact if accepted in the state - Required
• BLS Certification from AHA or American Red Cross - Required

Nice To Haves

• BSN or an equivalent – Highly preferred

Responsibilities

• Provide Nursing Care for Hospice Patients in private homes, nursing homes, group homes or assisted living facilities.
• Manage assigned caseload of patients and lead the interdisciplinary team caring for those patients.
• Provide assessments, care planning, pain management and symptom management and all other aspects of hospice nursing.
• Administer Physician-prescribed medication.
• Ensure timely and accurate submission of patient documentation and charting in adherence to client policies.
