About The Position

Crest Palliative Care is seeking a compassionate Registered Dietitian to join its interdisciplinary hospice team. The Dietitian provides nutritional assessments, recommendations, and education for hospice patients with complex medical conditions. In collaboration with physicians, nurses, social workers, chaplains, and hospice aides, the role helps patients and families make informed decisions that support comfort and dignity and align with each patient’s goals and hospice plan of care. The position includes consultations in patients’ homes, facilities, and assisted living settings.

Requirements

Current Texas Registered Dietitian credential.
Bachelor's degree in Dietetics, Nutrition, or a related field.
Excellent communication and patient education skills.
Valid driver's license and reliable transportation.

Nice To Haves

Minimum one year of clinical experience preferred.
Experience in hospice, home health, geriatrics, oncology, or palliative care preferred.

Responsibilities

Conduct nutritional assessments and identify nutrition-related needs.
Develop individualized nutrition recommendations consistent with hospice goals of care.
Collaborate with the interdisciplinary team regarding patient care planning.
Educate patients, caregivers, and staff on nutrition, hydration, symptom management, and comfort-focused feeding approaches.
Document assessments and recommendations according to agency standards.
Participate in interdisciplinary team meetings as needed.
Provide consultation for patients in homes, facilities, and assisted living settings.
