About The Position

This is a full-time, 40-hour-per-week day-shift position serving patients in facilities and homes in Centerville, Ohio, and surrounding areas. The role involves driving to patient locations. As a Hospice PCS, you will work alongside an interdisciplinary team to provide compassionate care that enhances patients’ quality of life and supports families, bringing comfort, dignity, and peace.

Requirements

High school diploma or GED.
Computer skills sufficient to properly document services and care.
Ability to drive during daytime, nighttime, or inclement weather.
Valid driver’s license with a safe driving record.
State-minimum automobile insurance coverage.

Nice To Haves

Certified Nursing Assistant certification with the State of Ohio.
One year of experience.
Certification in Hospice & Palliative Medicine (CHPNA), preferred within two years of employment.
