About The Position

The Hospice Patient Care Coordinator role is necessary for the implementation, evaluation, and coordination of the plan of care for hospice patients. This role coordinates treatment activities, gathers information for assessments and ongoing assessment of care plans. This position ensures continuity of care, provides information, education, and advocacy for the care of the patient. WHAT YOU CAN EXPECT Serves as a resource for staff regarding applicable regulations, policies and procedures. Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Conducts audits, identifies corrective action, and initiates follow-up to ensure compliance with established policies and regulations. Conducts home visits for the purpose of supervising assigned staff and identifies potential deviations intervening when deviations from established practice are identified. Coordinates staffing schedule to provide volume and patient-specific need-based staffing; adapts schedule to adjust for staff shortages ensuring adequate staffing for quality patient care. Ensures and plans the delivery of high-quality clinical services. Implements, monitors, and analyzes variances of the Quality Improvement Program. Investigates, evaluates, and resolves concerns of those we are privileged to serve and keeps the next level of management informed of significant developments. Participates in people management activities for direct team members such as scheduling, providing feedback for performance evaluations, disciplinary actions and interviews. Participates in monitoring patient satisfaction data and identifies opportunities to enhance the patient experience. Serves in management on-call rotation to support off shift, weekend and holiday patient care. Supervises the work of others and manages the performance of individuals through feedback and recommendations. Trains and educates patients and family members or other support and documents understanding of treatment plan and goals.

Requirements

  • Associate's Degree- Nursing- Required
  • 2 Years Nursing Experience- Required
  • Registered Nurse (RN)- State Licensing Board- Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
  • Driver's License- Required
  • Auto Insurance- Required

Nice To Haves

  • Bachelor's Degree- Nursing- Preferred
  • 2 Years Home Health or Hospice Experience- Preferred
