About The Position

Silverado Palliative Coastal Valleys is hiring a Hospice Licensed Vocational Nurse to provide skilled nursing and palliative care to patients and families facing life-limiting illnesses. The role covers San Bernardino County, Riverside County, and Orange County, with travel to patients’ homes. The LVN provides care under the direction of a Registered Nurse and collaborates with the interdisciplinary Palliative team to support patients and families with empathy, professionalism, and dignity.

Requirements

• Valid LVN/LPN license in the state.
• Minimum 1 year of LVN/LPN experience.
• CPR certification.
• Strong written and verbal communication skills.
• Comfort working around animals and pets.
• Valid driver’s license and clean driving record.
• Reliable transportation and willingness to travel.
• May be required to work outside regular hours based on patient needs.

Nice To Haves

• Hospice experience preferred but not required.

Responsibilities

• Deliver skilled nursing care under the direction of a Registered Nurse.
• Provide pain and symptom management, medical treatments, and patient education.
• Support patients and families through end-of-life care with empathy and professionalism.
• Participate in care planning and collaborate with the interdisciplinary Palliative team.
• Maintain accurate documentation and comply with Palliative protocols.
• Travel to patient homes across San Bernardino County, Riverside County, and Orange County.
