About The Position

Silverado Palliative Coastal Valleys is hiring a Hospice Licensed Vocational Nurse to provide skilled nursing and palliative care to patients and families across San Diego County. The role combines clinical expertise, empathy, and relationship-building to support patients facing life-limiting illnesses and their families with dignity. The LVN provides care under the direction of a Registered Nurse and collaborates with the interdisciplinary Palliative team. This is a PRN (per diem) position with variable shifts as needed; the listed schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m. The position requires travel to patient homes and may require work outside regular hours based on patient needs.

Requirements

Hold a valid LVN/LPN license in the state.
Have a minimum of one year of LVN/LPN experience.
Hold CPR certification.
Have strong written and verbal communication skills.
Be comfortable working around animals and pets.
Have a valid driver's license and clean driving record.
Have reliable transportation and be willing to travel.
May be required to work outside regular hours based on patient needs.

Nice To Haves

Hospice experience is preferred but not required.

Responsibilities

Deliver skilled nursing care under the direction of a Registered Nurse.
Provide pain and symptom management, medical treatments, and patient education.
Support patients and families through end-of-life care with empathy and professionalism.
Participate in care planning and collaborate with the interdisciplinary Palliative team.
Maintain accurate documentation and comply with Palliative protocols.
Travel to patient homes across San Diego County.
