About The Position
Hopestone Hospice is seeking a PRN Hospice Field Registered Nurse (RN) to deliver compassionate, one-on-one care to patients facing life-limiting illnesses, primarily in their homes. This role offers autonomy in clinical decision-making while providing support from a collaborative interdisciplinary team. It is ideal for an experienced hospice RN who values flexibility, independence, and meaningful work.
Requirements
- Active Registered Nurse (RN) license
- CPR certification
- Valid driver’s license and auto insurance
Nice To Haves
- Minimum 1 year of hospice experience preferred (2+ strongly preferred)
- Experience with Homecare Homebase is a plus
Responsibilities
- Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs.
- Determine hospice eligibility, admission, and recertification using established clinical criteria.
- Develop, implement, and adjust individualized plans of care to ensure comfort and quality of life.
- Provide education and emotional support to patients and families on disease progression, end-of-life care, and self-care techniques.
- Coordinate with the interdisciplinary group (IDG) including physicians, social workers, and chaplains.
- Assess caregiver capability and identify support needs throughout the care journey.
- Supervise and provide clinical direction to Hospice Aides and LVN/LPNs.
- Document timely and accurate clinical and progress notes.
- Communicate changes in patient condition and collaborate on care adjustments.
