Requirements
- Current CNA certification (required)
- Active CPR certification
- Valid Driver’s License and auto insurance
- Reliable transportation
Nice To Haves
- Hospice or home health experience preferred
- Experience with HomeCare HomeBase is a plus
- Compassionate and patient-centered
- Strong communication skills
- Dependable and self-motivated
- Comfortable working independently in home settings
Responsibilities
- Provide compassionate, hands-on personal care to hospice patients in their homes
- Assist with bathing, grooming, dressing, and hygiene
- Support safe transfers and ambulation
- Monitor and report changes in patient condition to the RN
- Maintain accurate, timely documentation
- Help ensure a clean, safe, and comfortable environment
- Support patients with nutrition and hydration needs
- Communicate respectfully with patients, families, and care team members
- Follow infection control and safety protocols
