About The Position

This role is for a relationship-driven healthcare professional focused on connecting with providers, growing referral networks, and making a difference in patients’ lives. At Choice Hospice, the position involves helping patients and families access compassionate, end-of-life care. It is not a desk job; the liaison will be out in the community, building meaningful partnerships with physicians, hospitals, and care teams, becoming a trusted resource. The ideal candidate is motivated by results, relationships, and purpose.

Requirements

• Background in healthcare (sales, clinical, or referral coordination)
• Skilled at building relationships and influencing decisions
• Driven, self-motivated, and goal-oriented
• Comfortable working in the field and managing your own territory
• Valid Driver’s License
• Valid Auto Insurance

Nice To Haves

• Minimum of 2 years home health or hospice sales experience
• Hospice or home health experience
• HomeCare HomeBase experience

Responsibilities

• Build and grow strong referral relationships across hospitals, SNFs, ALFs, and physician offices
• Position Choice Hospice as the provider of choice through education and trust
• Identify new opportunities and expand your territory
• Collaborate with clinical teams to ensure seamless patient transitions
• Represent the organization at community events and networking opportunities
• Meet and exceed monthly admission goals
