Hospice Certified Nursing Assistant (CNA)
About The Position
The Certified Nursing Assistant (CNA) works under the direction of a Case Manager/DON and provides resident care in accordance with resident assessments and care plans, as directed by nursing management. The CNA works with other licensed care staff to coordinate appropriate care and follows company nursing policies, procedures, and safety guidelines. Suncrest Hospice aims to provide exceptional care and service to patients by increasing staff-to-patient ratios, clinical visits, and support for clinicians. The company is a Community Health Accreditation Partner (CHAP)-certified hospice.
Requirements
- Strong commitment to improving quality of life while partnering with community organizations in patient care.
- Ability to collaborate effectively in a team environment.
- Ability to work independently on a day-to-day basis.
- Willingness to drive; reliable transportation, a valid driver’s license, and auto insurance are required.
- Certified Nursing Assistant (CNA) certification in the state(s) of practice.
- CPR certification.
Nice To Haves
- Interest in pursuing certifications and further education.
- Ability to work one-on-one with patients while drawing on support from a patient care team.
Responsibilities
- Provide resident care according to resident assessments and care plans and as directed by nursing management.
- Coordinate appropriate care with other licensed care staff.
- Follow company nursing policies and procedures and safety guidelines.
- Work collaboratively with nurses, social workers, chaplains, volunteers, and CNA leadership.
