About The Position
University Home Care Inc is seeking a dedicated Hospice Case Manager to join its team in Oakland, MI. The role is suited to a Registered Nurse (RN) with hospice experience who is committed to providing care and support to patients and their families during end-of-life care. The case manager conducts patient assessments, develops and adjusts individualized care plans, coordinates with interdisciplinary teams, and provides education and emotional support. The role also requires travel to Macomb and Oakland County and maintaining documentation that meets regulatory and company standards.
Requirements
- Valid Registered Nurse (RN) license.
- Previous hospice nursing experience required.
- Strong clinical assessment and care planning skills.
- Ability to travel to Macomb and Oakland County.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Coordinate and communicate effectively with interdisciplinary teams, patients, and families.
- Monitor patient progress and adjust care plans as needed to ensure quality and compliance.
- Provide education and emotional support to patients and their families throughout the hospice journey.
- Ensure all documentation meets regulatory and company standards.
