About The Position

As a Per Diem RN Case Manager, you will coordinate individualized hospice care by assessing patients, working with the interdisciplinary care team, and developing plans that support patient needs. Requirements
- Current RN license specific to the state or states assigned.
- Current CPR certification.
- At least 1 year of nursing experience.
- Valid driver's license, access to reliable transportation, and liability insurance.
- Must pass a drug test before beginning employment.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately and on time, in accordance with care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
