This role is responsible for expanding market presence, developing strategic referral partnerships, and driving sustainable census growth through meaningful relationships and exceptional service. Requirements

  • Minimum 2 years of Home Health or Hospice Business Development/Sales experience strongly preferred
  • Proven success building referral relationships and driving admissions growth
  • Knowledge of hospice eligibility and healthcare referral processes preferred
  • HomeCare HomeBase experience preferred
  • Valid Driver’s License and current Auto Insurance
  • Strong communication, presentation, and relationship-building skills

Responsibilities

  • Develop and maintain strong referral relationships with physicians, hospitals, case managers, SNFs, assisted living communities, and other community partners.
  • Present Choice Hospice’s care model in a way that positions us as the provider of choice in the market.
  • Identify and cultivate new business opportunities to increase hospice admissions and market growth.
  • Serve as a knowledgeable resource and advocate for hospice services and patient eligibility.
  • Partner closely with clinical and operations teams to support seamless patient transitions and continuity of care.
  • Create and execute strategic territory growth plans.
  • Establish community outreach and public relations initiatives that strengthen brand awareness.
  • Maintain full compliance with local, state, and federal healthcare marketing regulations.
  • Consistently meet and exceed monthly admission and growth goals.
