About The Position

The Choice Hospice Aide provides patient care, including assistance with personal healthcare and related services in the patient’s residence. The aide helps maintain a safe and clean environment, works cooperatively with patients and families, and shares observations and problems with the supervisor. Hospice Aides are part of the care team supporting the hospice mission.

The role provides direct care under the direction of an RN and according to the Aid Plan of Care. It includes helping patients with safe transfers, ambulation, nutrition, and fluid intake; performing treatments and procedures according to agency policy; following infection-control principles; documenting care and reporting changes; and promoting respectful communication, privacy, and quality care. The aide also contributes to efficient agency operations and uses effective time management.

Requirements

• Current CNA/HHA and LTC certifications.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aid Plan of Care.
• Assist patients with safe transfers and ambulation according to agency policy.
• Report patient changes and document pertinent information and care provided to ensure continuity of care.
• Follow accepted infection-control principles and provide a clean, safe, and comfortable environment.
• Promote positive, supportive, and respectful communication with patients, families, and other employees.
• Promote respect for patients, their privacy, and their property.
• Assist patients with proper nutrition and adequate fluid intake.
• Contribute to efficient agency operations and demonstrate effective time management.
• Perform treatments and procedures according to agency policy.
• Promote the agency philosophy and administrative policies to ensure quality of care.
