About The Position

The Choice Hospice Aide provides patient care, including assistance with personal healthcare and related services, in patients’ places of residence. The aide helps maintain a safe and clean environment, works cooperatively with patients and families, and shares observations and problems with the supervisor. Working under the direction of an RN and according to the Aid Plan of Care, the Hospice Aide supports the hospice care team and the agency’s mission.

Requirements

• Current CNA/HHA and LTC certifications.
• Valid driver’s license.
• Valid auto insurance.
• CPR certification.

• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aid Plan of Care.
• Assist patients with safe transfers and ambulation in accordance with agency policy.
• Report changes appropriately and document pertinent information and care provided to support continuity of care.
• Practice accepted infection control principles.
• Provide a clean, safe, and comfortable environment.
• Communicate positively, supportively, and respectfully with patients, families, and other employees.
• Promote respect for patients, their privacy, and their property.
• Assist patients with proper nutrition and adequate fluid intake.
• Contribute to the agency’s management and efficient operation, demonstrating effective time management.
• Perform treatments and procedures according to agency policy.
• Promote the agency philosophy and administrative policies to help ensure quality of care.
