Hospice Aide PRN
About The Position
The Choice Hospice Aide provides care to patients in their place of residence, including personal healthcare and related services, and helps maintain a safe, clean environment. The aide works cooperatively with patients and families and shares observations and problems with the supervisor. Hospice Aides are a key part of the care team and contribute to delivering the hospice mission. The role provides direct patient care under the direction of the RN and according to the Aid Plan of Care. It also supports patient safety, comfort, privacy, nutrition, and continuity of care while contributing to the efficient operation of the agency.
Requirements
- Current CNA/HHA and LTC certifications.
- Valid driver’s license.
- Valid auto insurance.
- CPR certification.

- HomeCare HomeBase experience preferred.
Responsibilities
- Provide direct patient care under the direction of the RN and according to the Aid Plan of Care.
- Assist patients with safe transfers and ambulation in accordance with agency policy.
- Report changes and document pertinent information and care provided to support continuity of care.
- Practice accepted infection-control principles.
- Provide a clean, safe, and comfortable environment.
- Promote positive, supportive, and respectful communication with patients, families, and other employees.
- Promote respect for patients, their privacy, and their property.
- Assist patients with proper nutrition and adequate fluid intake.
- Contribute to the management and efficient operation of the agency and demonstrate effective time-management skills.
- Perform treatments and procedures according to agency policy.
- Promote the agency philosophy and administrative policies to ensure quality of care.
- Share observations and problems with the supervisor.
