About The Position

The Hospice Aide is a Certified Nursing Assistant and paraprofessional member of the interdisciplinary care team who works under the supervision of a Registered Nurse. The aide provides direct support and personal care to hospice patients, assisting with day-to-day needs to provide comfort and enhance quality of life. The role follows each patient’s individual Plan of Care, provides in-person care, maintains documentation, and coordinates with the patient’s RN Case Manager. The position is full-time and based in Nixa, Missouri, with 100% travel.

Requirements

Active Certified Nursing Assistant (CNA) license in the state of practice.
At least 18 years of age.
Ability to pass a background study, consumer report, and drug test.
Valid state driver’s license, access to an automobile in good working order, automobile insurance that meets state and organizational requirements, and an acceptable driving record.
Ability to push, pull, lift, and carry up to 30 pounds, and occasionally up to 50 pounds.
Ability to adapt to a variety of work conditions to provide quality patient care in a home setting.

Responsibilities

Provide personal care, including bathing, dressing, oral hygiene, and changing bed linens.
Assist patients with mobility, transfers, and exercises as instructed by nursing staff.
Assist with meals, feeding, and assigned light household tasks.
Monitor and report changes in patient condition to the RN Case Manager.
Provide routine skin care to prevent breakdown and support patient comfort.
Provide medication reminders in accordance with state regulations.
Maintain a clean, safe, and organized patient environment.
Provide emotional support and respite for family caregivers.
Use equipment safely and appropriately for patient care needs.
Follow documentation and HIPAA compliance standards.
Participate in quality assessment and performance improvement activities.
Perform other duties as assigned.
