About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Hospice Aide - CNA provides direct patient care under the direction of an RN and in accordance with the Hospice Aide/Homemaker Plan of Care. The role also supports safe patient transfers and ambulation, communicates and documents changes and care provided, follows infection-control principles, and promotes respectful communication with patients, families, and colleagues.

Requirements

• High school diploma preferred or GED.
• Unencumbered certification according to the applicable Nurse Aide Registry.
• Current CNA license.
• At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Demonstrated interest in the welfare of ill and elderly people and a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.
• Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.

• EMR experience.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Assist patients with safe transfers and ambulation according to agency policy.
• Report and communicate changes, and document pertinent information and care provided to ensure continuity of care.
• Practice accepted infection-control principles.
• Promote positive, supportive, and respectful communication with patients, families, and other employees.
• Submit all documentation in a timely manner.
• Perform other duties as required.
• Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
