About The Position

Heart’n Soul Hospice is seeking a Hospice Aide - CNA to provide compassionate direct care to hospice patients and support their families. The role works under the direction of an RN and follows the Hospice Aide/Homemaker Plan of Care. The position requires an active, unencumbered CNA certification, relevant care experience, dependable transportation, and a commitment to patient dignity, confidentiality, and high-quality care.

Requirements

High school diploma preferred or GED.
Unencumbered certification according to the applicable Nurse Aide Registry.
Current CNA license.
At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
Demonstrated interest in the welfare of ill and elderly people and a compassionate approach to care.
High integrity, including maintaining confidential information.
Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.

Responsibilities

Provide direct patient care under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
Assist patients with safe transfers and ambulation in accordance with agency policy.
Report and communicate changes and document pertinent information and care provided to support continuity of care.
Practice accepted infection control principles.
Promote positive, supportive, and respectful communication with patients, families, and other employees.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
