About The Position

As an admissions nurse at VITAS Healthcare, you are often the first face of hospice for patients and families. You may lead their first in-depth discussion about end-of-life care, helping set expectations for the journey ahead. You travel within your assigned service area to meet patients and families at hospitals, nursing facilities, and patient residences, assessing each patient’s need for care as calls arrive. Working alongside your team, you help deliver compassionate, personalized care with empathy while honoring each patient’s wishes.

Requirements

• Current RN license.
• Reliable transportation.
• Previous experience in a physician’s office, hospital, long-term care facility, or assisted living facility.

Nice To Haves

• Bachelor’s degree preferred.

Responsibilities

• Travel to meet with patients and families.
• Educate patients and families about the benefits of hospice care and services offered by VITAS.
• Ask questions and listen empathetically to understand each patient’s end-of-life needs, wants, and beliefs.
• Use information gathered to determine whether hospice is the right path for the patient.
• Ensure a comfortable transition for patients who elect to receive hospice care with VITAS.
• Travel within the assigned service area to hospitals, nursing facilities, and patient residences to assess patients’ need for care.
