Hospice Admissions RN
About The Position
As an admissions nurse at VITAS Healthcare, you are often the first face of hospice for patients and families. You meet with them for an in-depth discussion about end-of-life care, helping set expectations for the journey ahead. You travel within your assigned service area to hospitals, nursing facilities, and patient residences to assess patients’ needs for care. Most admissions nurses work four 10-hour shifts per week, although schedules may vary. You educate patients and families about hospice care and VITAS services, listen empathetically to learn about each patient’s end-of-life needs, wants, and beliefs, and use that information to determine whether hospice is appropriate. When patients elect VITAS hospice care, you help ensure a comfortable transition. You work alongside your team to provide compassionate, personalized care that honors each patient’s wishes.
Requirements
- Current RN license.
- Reliable transportation.
- Previous experience in a physician’s office, hospital, long-term care, or assisted living facility.
Nice To Haves
- Bachelor’s degree preferred.
Responsibilities
- Travel to meet with patients and families within the assigned service area.
- Educate patients and families about the benefits of hospice care and the services offered by VITAS.
- Ask questions and listen with empathy to understand each patient’s end-of-life needs, wants, and beliefs.
- Determine whether hospice is the right path for the patient based on the information gathered.
- Ensure a comfortable transition for patients who elect hospice care with VITAS.
- Travel to hospitals, nursing facilities, and patient residences to assess patients’ needs for care.
