About The Position

As an admissions nurse at VITAS Healthcare, you are the first face of hospice for patients and families. Your initial meeting may be their first in-depth discussion about end-of-life care, and your approach helps set expectations for the journey ahead. You travel within an assigned service area to meet patients and families and assess patients’ needs for care at hospitals, nursing facilities, and patient residences. Working with your team, you help provide compassionate, personalized care that honors each patient’s wishes. Most admissions nurses work four 10-hour shifts per week, though schedules may vary.

Requirements

• Current RN license.
• Reliable transportation.
• Previous experience in a physician’s office, hospital, long-term care, or assisted living facility.

Nice To Haves

• Bachelor’s degree preferred.

Responsibilities

• Travel to meet with patients and families.
• Educate patients and families about the benefits of hospice care and the services offered by VITAS.
• Ask questions and listen empathetically to understand each patient’s end-of-life needs, wants, and beliefs.
• Use gathered information to determine whether hospice is the right path for the patient.
• Ensure a comfortable transition for patients who elect to receive hospice care with VITAS.
• Travel within the assigned service area to assess patients at hospitals, nursing facilities, and patient residences.
