The position also involves evaluating the need for continued services for patients readmitted to the hospital or post-acute settings and potentially participating in precepting new hires. A significant aspect of the role may include managing hospice admissions, including obtaining history/physical, implementing care plans, and managing symptoms of terminal diagnoses to promote end-of-life comfort. Collaboration with the business development team to achieve admission growth and increase market share is also expected.
