About The Position

The Admissions Nurse at Lifted Hospice serves as the first clinical touchpoint for patients and families entering hospice care. This role involves guiding individuals through the admission process with clarity, compassion, and clinical excellence. The Admissions RN completes initial assessments, provides skilled nursing care, and educates patients and families about what to expect during their hospice journey. Collaboration with the interdisciplinary care team is essential to ensure a smooth transition and a personalized plan of care from the outset. The position aims to provide peace, support, and comfort, setting a positive tone for the care ahead. The role is crucial in representing 'The Lifted Way' by ensuring patients are welcomed with dignity and families feel heard and supported, initiating every admission with excellence.

Requirements

  • Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice.
  • At least two years of experience as a registered nurse in a clinical care setting.
  • Strong assessment, critical thinking, and communication skills — able to guide urgent and emotional conversations with calm and clarity.
  • Familiarity with state and federal hospice regulations (including eligibility criteria and CoPs).
  • Ability to work independently, prioritize needs quickly, and make decisions under pressure.
  • Comfortable navigating emotional, logistical, and cultural complexities during admissions.
  • Able to meet documentation deadlines while supporting patient and family needs.
  • Must be able to travel locally to complete admissions across homes and facilities.
  • Current driver’s license, reliable transportation, and valid auto liability insurance.
  • Must be able to read, write, and communicate clearly in English.

Nice To Haves

  • Hospice experience preferred.

Responsibilities

  • Completing timely and thorough nursing assessments to identify each patient’s physical, emotional, and environmental needs at admission.
  • Developing and documenting a personalized care plan in coordination with the patient, family, and interdisciplinary team.
  • Delivering compassionate, clinically excellent nursing care at the start of care, using proper infection control measures in line with OSHA standards.
  • Collaborating closely with the medical director, physicians, hospice leadership, and the full interdisciplinary team to ensure patient and family needs are fully understood and supported.
  • Submitting all required documentation and admission paperwork accurately and on time, maintaining compliance with all regulatory standards.
  • Offering education, comfort, and clear communication to help families understand the hospice journey and feel confident in what’s ahead.
  • Participating in on-call rotations and providing patient care and support after hours as assigned.
  • Participating in ongoing training, hospice in-services, and quality improvement (QAPI) initiatives to maintain clinical excellence and professional growth.
