About The Position

Envision Hospice is seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments, primarily serving patients throughout King County. The nurse evaluates patient eligibility, reviews medical histories, coordinates with the care team, and provides education and emotional support to patients and families. Care is provided in private homes, adult family homes, assisted living communities, and skilled nursing facilities. The position is based in King County, with occasional travel to Snohomish, Kitsap, Thurston, and Pierce counties as needed.

Requirements

• Active Washington State RN license and BLS certification.
• Valid driver’s license and reliable transportation.
• Minimum 2–3 years of healthcare-related experience required.
• Proficiency with EMR systems and healthcare technologies.
• Strong judgment, initiative, and ability to work independently.
• Cultural sensitivity and adaptability to a diverse patient population.

Nice To Haves

• Minimum 1 year of hospice or home health nursing experience preferred.

Responsibilities

• Conduct patient assessments addressing physical, emotional, and spiritual needs.
• Collaborate with physicians and the interdisciplinary team on care plans.
• Educate and support patients and families during hospice admission.
• Complete required documentation accurately and promptly.
• Perform hospice admissions and, when not scheduled, provide routine visits, including death visits.
• Stay current with hospice best practices, policies, and regulatory standards.
