About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this role, the Nurse Case Manager guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The position includes patient assessments and care planning, referrals to other disciplines, patient education and support for self-care, supervision of LPNs and HHAs, accurate and timely documentation, and communication with the clinical manager and care team. The nurse also plans and provides staff education, performs assigned on-call responsibilities, and participates in clinical development and continuing education programs. Other duties may be assigned.

Requirements

Current RN license specific to the state or states assigned.
At least 1 year of clinical experience as a Registered Nurse.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on patients’ identified needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
