About The Position

Amedisys is hiring a Home Health Physical Therapist serving Middle to Lower Westchester County. The role focuses on helping patients maintain, restore, and improve movement, activity, and function in the home setting. The therapist provides interventions, conducts reexaminations at least every 30 days, adjusts interventions as needed, and performs discharge planning. Other duties may be assigned.

Requirements

• Current license to practice physical therapy in the state where assigned to work.
• Graduation from a physical therapy educational program accredited by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year of related physical therapy experience for physical therapists entering home health care. Applicants with less than one year of experience require approval from regional clinical leaders and appropriate mentoring and/or participation in a residency program.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions and conduct reexaminations at least every 30 days.
• Modify interventions as necessary.
• Perform discharge planning.
• Carry out other duties as assigned.
