About The Position

AHN Healthcare@Home is hiring a Physical Therapist to provide skilled, patient-centered care to patients in their homes in the Uniontown, Pennsylvania area. The role offers autonomy, meaningful patient relationships, and career growth, with collaboration from an interdisciplinary team. It is eligible for a $10,000 first-year success bonus tied to key achievements, with additional bonus opportunities as employees grow with the organization.

The Physical Therapist evaluates patients, assesses functional mobility, and develops individualized therapy plans to improve strength, balance, and independence. The role includes educating patients and caregivers, documenting care in the EMR, and traveling independently to patient homes within an assigned territory. A field education team provides support from the first day, alongside leadership and a collaborative culture.

Requirements

• Hold an active Physical Therapist license in the state where the employee works.
• Have at least one year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
• Have a valid driver’s license.
• Be able to spend approximately 20% of the workday driving to and from patient locations.
• Successfully complete a drug screen prior to employment.
• Be legally authorized to work in the United States.

Responsibilities

• Deliver skilled, patient-centered care in patients’ homes in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations and functional mobility assessments.
• Develop individualized therapy plans to improve patients’ strength, balance, and independence.
• Educate patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.
• Use critical thinking and problem-solving skills in real time.
• Complete thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
