Home Health RN | Patient Visit Nurse
About The Position
Home Health Care, Inc. is seeking a Full-Time RN Visit Nurse to provide skilled nursing care throughout Mankato and surrounding Southern Minnesota communities. This role involves traveling to patients' homes to deliver essential healthcare services, assess their condition, manage medications, provide treatments, and educate patients and caregivers. The position offers a unique opportunity to see the whole patient picture in their home environment and utilize nursing judgment to improve outcomes, reduce hospital readmissions, and help patients remain independent. The role is ideal for an experienced RN who enjoys autonomy, problem-solving, patient education, and clinical decision-making.
Requirements
- An active Minnesota Registered Nurse (RN) license
- Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
- Strong assessment and critical-thinking skills
- Confidence making clinical decisions in the field
- Excellent patient and caregiver communication
- Strong organization and time-management abilities
- Ability to manage a daily schedule of patient visits
- Reliable transportation and willingness to travel throughout the Southern Minnesota service area

Responsibilities
- Provide skilled RN home health visits throughout your assigned territory
- Complete comprehensive nursing assessments
- Perform wound assessment and wound care
- Provide medication reconciliation and management
- Administer injections and other skilled nursing treatments
- Monitor acute and chronic medical conditions
- Identify and respond to changes in patient condition
- Educate patients and caregivers about medications, diagnoses, treatments, and recovery
- Coordinate care with physicians and healthcare providers
- Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
- Support patients transitioning home following hospitalization or rehabilitation
- Help reduce avoidable emergency department visits and hospital readmissions
- Complete timely documentation using Homecare Homebase (HCHB)
